Our electronic health record, TherapyNotes has a client portal which allows clients to login online and fill out all required forms: client history, consent for services, notice of privacy practices, insurance and demographic information forms, clinical questionnaires, etc. ​

If you are a current client, you will receive a "welcome e-mail" from TherapyNotes with a link to create your username and password. If you ever need your password reset, please notify your clinician. The link below will take you directly to the client portal page where you can complete documentation for services.
